Holistic support for birthing people and families navigating complex life challenges.
At ITAV Wellness, our Village Care Case Management program provides wraparound support to pregnant and postpartum families—especially those impacted by housing instability, medical complications, mental health concerns, justice involvement, or child welfare systems.
We take a healing-centered, culturally rooted, non-judgmental approach that honors the lived experiences of Black and Brown families in Los Angeles County.
WHAT WE OFFER
- Comprehensive intake & needs assessment
- Warm handoffs to housing, behavioral health, medical, and social services.
- Support navigating DCFS involvement
- Resource linkage (food, diapers, transportation, insurance, benefits)
- Safety planning
- Goal setting and care planning
- Consistent check-ins via phone, text, or in person
- Advocacy during appointments and case meetings
WHO THIS SERVICE IS FOR
- Pregnant and postpartum Mothers(up to 24 months postpartum)
- Fathers who are primary caregivers.
- Families experiencing housing insecurity.
- Families impacted by child welfare.
- Youth parents.
-Families needing coordinated support from multiple systems.
